Registration for the Mission Trail Poodle Club 48th and 49th Specialty Shows (2011)
CLOSING IS NOON, OCTOBER 12, 2011

Download and complete the the 2011 Fill-in Entry Form here. You can then:

  1. FAX your entry to: +1-408-578-8378 (Attn: Carol Kent),
  2. Mail it to: Carol Kent, 6130 Monterey Hwy. #92, San Jose, CA 95138-1713, or
  3. Email it to: cmkent@att.net.

To submit your entry electronically, follow the directions below:

  1. Download and save the form to your computer (remember where you saved it).
  2. Fill in the form & save again (this is your copy).
  3. Send email to Carol Kent with entry/entries as an attachment.
  4. Mail check to Carol Kent, 6130 Monterey Hwy. #92, San Jose, CA 95138-1713 if there is enough time before the closing date, or use PayPal to pay online using your credit card or PayPal balance.
  5. If you selected PayPal as your payment option, email a copy of your electronic receipt to Carol Kent.
